Position Overview & Specifications

TRANSITION SUPPORT WORKER

Job Type - Permanent/Part-Time

Hours - 18.5 Hours Per Week (3-week rolling rota; 4 shifts on, 4 shifts off 5.45pm -11pm)

Salary - £10,486.26 per annum (Base Salary £10,023.76 + £462.5Unsociable Hours allowance)
Location - Lincoln

Are you looking for a new role where you can make a difference every day? Do you want to work as part of an established team that employs a non-judgemental approach, sensitivity and understanding? This might be the right role for you!

This role is based in our friendly and supportive team in our Young People's Hub in Lincoln. You will work with Young People aged 16-21 who are homeless or are threatened with homelessness and have suppport needs including social, emotional, and behavioural difficulties. Your role will be to support them to transition from supported accommodation into more independent accommodation and develop life skills along the way. You will provide advice and assistance to these Young People to support them with becoming ambitious and independent and embrace new opportunities.

Key Responsibilities

  • Have an ongoing oversight of the exit date from the service for each Young Person at the point of their move in and communicate regularly with the team to pre-empt when a Young Person is ready to move out of service.
  • Act as the first point of contact for the yYoung People within the transition period to move on phase and offer ad-hoc advice, guidance and reassurance.
  • Support the Young Person to apply for move on options and to arrange and attend any viewings of move on properties.
  • Complete necessary budgeting work, depending on the needs of the Young Person.
  • Review and update the Young Person’s move on plan and complete regular joint meetings with the keyworker and any other external agencies involved.
  • Complete home visits to offer any further support required.
  • Host weekly drop ins to ensure access to support for those who have left service.

What we need from you

  • To be approachable and have the ability to build rapport and trusted relationships with the Young People, colleagues and outside agencies.
  • An understanding of the support needs of Young People who typically use our services.
  • Knowledge and understanding of housing provision and the welfare benefits system.
  • Previous experience of working with Young People or extensive experience of providing care and/or support services within another social care setting.
