About the Role

Great Yarmouth and Northern Villages Primary Care Network has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join our primary care network, to help us to build on the work already started, to improve the quality of care and reduce health inequalities, across our deprived area.

We would be happy to consider the right applicant on a full-time or part-time basis, and there is an option to include some home or remote working. The successful candidate will be developed, managed and mentored by the senior clinical pharmacist.

Our pharmacy team, consisting of pharmacists, pharmacy technicians and pharmacy care-coordinators, provides expert medicine-optimisation throughout the network, as well as supporting practice-based clinicians with medication issues. This is well-established, yet evolving, team is a vital part of the chronic disease-management team and conducts clinical reviews of selected patients with the support of GP and nursing mentors. The main responsibilities of this role include:
  • working with the multidisciplinary team, to undertake structured clinical medication reviews, proactively managing people with complex polypharmacy;
  • providing specialist expertise in the use of medicines, whilst helping to address both the public health and social care needs of people living within the network area;
  • providing guidance on person-centred medicines optimisation;
  • through structured medication reviews and shared decision-making, supporting patients to achieve the best outcome for them, reducing waste and promoting self-care, where possible.

About the Candidate

This role is pivotal to improving the quality of patient care and operational efficiencies of the PCN, so requires someone with the motivation and the passion to deliver excellent service within general practice. All applicants should have:
  • A degree in pharmacy;
  • Registration with the General Pharmaceutical Council;
  • Qualified independent prescriber, or working towards this;
  • Member of the Royal Pharmaceutical Society (RPS);
  • Post-qualification experience, ideally in primary care;
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Basic life-support training;
  • Safe-guarding adult and children, level three.

The Great Yarmouth and Northern Villages Primary Care Network delivers high quality, patient-centred integrated care service to improve outcomes for our local population, bringing together local health and care organisations (including social care, the voluntary sector and patient groups) to work collaboratively as part of the NHS Long Term Plan. The successful candidate will be employed by The Park Surgery and will work on behalf of Great Yarmouth and Northern Villages Primary Care Network, which consists of 4 practices: The Park Surgery, East Norfolk Medical Practice, Coastal Partnership and Fleggburgh Surgery.
