Position Overview & Specifications
You will join the Integrated Care Service (iCares) which works across Sandwell providing rehabilitation, case management and admission avoidance to people in their own homes. These are often people recovering from an illness, a hospital stay or increasing frailty. With our support and specialist rehabilitation we aim to improve their physical function such as walking, looking after themselves and even returning to community activities again.
Within the Team we have Occupational Therapists, Physiotherapists, Community Matrons, Speech and Language Therapists, District Nurses, and Assistant Practitioners all working together to deliver integrated care.
