Position Overview & Specifications

CERT was set up in 2014 to provide intensive, bespoke packages of care to service users in their own homes, as an alternative to being in Locked Rehabilitation, out of city hospital placements. Over the past 7 years the team has expanded to work with up to 45 service users with mental health problems, personality disorders and/or learning disabilities to support them to move from hospital into their own tenancies. Referrals are primarily received from inpatient acute and rehabilitation services.

Service users are supported by a full multi-disciplinary team with the aim of enabling them to live independent and fulfilled lives. The team provides intensive quality interventions primarily between 8am and 9pm, 7 days a week with the ability to increase to 24 hours (where necessary) to promote recovery.

The Community Enhancing Recovery Team (CERT) is a service that provides individual packages of intensive quality intervention to a diverse group of people who are or have experienced difficulties due to complex health and social needs, in their own home.

As a recovery worker you will be working intensively with a small number of people, enabling them to live their lives and achieve their own goals with support from a Multi-Disciplinary team.

People who use the service are from diverse backgrounds and will often have mental health needs, learning disabilities or personality disorders. They will often have had long or frequent hospital admissions.

All team members will have the opportunity to contribute to the development of the team and we will offer induction and training in a wide range of areas which will give you the skills to feel confident in your role.

You will have the desire and passion to improve the quality of life for people in all aspects of their lives which may have been disrupted by ill-health.

The aim of CERT is to prepare people for the next step in their lives where we hope that people will require decreasing levels of support.

CERT is a service that provides a 7 day a week service between the hours of 8am and 9pm, the post holder would be required to work flexibly within this.

General Requirements

To be aware of and abide by service policies re confidentiality and information sharing

To have a working knowledge of Collaborative Care Planning

Receive supervision from agreed supervisor on a regular, planned basis, and work within the appraisal process

Receive team / peer supervision via attendance at weekly multidisciplinary team meeting

To be committed to professional development regarding practice of work with client group

To undertake general duties as part of the normal work of the team

To maintain accurate and up to date records of all contacts and interventions with service users

To undertake any other duties that maybe allocated by the Team Manager within the general scope of the post and appropriate to the grade.

It is the responsibility of all Trust employees to ensure that all duties are carried out in accordance with ‘best practice’ risk management.

To be able to work independently in Service Users homes.

Principal Duties

Implement agreed programmes of care with service users and carers under the supervision of a named professional.

Help to identify changes to an individuals mental health and have a good understanding and awareness of issues around risk.

To support service users in achieving their goals, by providing practical health and social care agreed by the client and their care co-ordinator

Be able to undertake basic physical health care observations.

Develop a rapport with service users within appropriate boundaries.

To take a role in health promotion both with service users, carers & other staff.

To take a role in evaluation of care provided

To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services.

Be able to refer individuals to appropriate ongoing community care services, including health, statutory organisations and voluntary services.

Engage in supervision and appraisal process with qualified team members

Service Development

To maintain existing skills and develop/receive training in new skills as required to carry out the work of the Trust.

Participate in service requirements for audit/data collection

Participate in training, team away days and induction plans
